The following information is required for the MacCHESS NIH Annual Progress Report and is crucial to our funding. 

There are 4 sections to this report, Co-Investigators, Funding, Progress Report and Publications.  Each section may be completed at different times, you do not need to do this all at once.

In order to complete these sections you will need to have:

1.  Names of all NEW co-investigators since June 2005 and their level of Degree

2.  Information on existing funding Grant/Contract #'s

3.  Your publications list

In each section is a Submit button.  The Submit button will take you on to the next section OR you may look at the next sections by clicking on the left side bar of that page. 

Information is NOT sent to CHESS nor saved until you click on the Submit button.
